what is baptism?
"Baptism is the way out of the kingdom of death into life, the gateway to the Church, and the beginning of a lasting communion with God." Baptism is the foundational Sacrament; it unites us with Jesus Christ and frees us from the power of Original Sin and personal sins and causes us to rise with Him to eternal life.
what is the FORm?
The classical form of Baptism is the threefold immersion of the baptismal candidate in water. However, usually, water is poured three times over the candidate's head. While the candidate is being blessed with the holy water, the minister says "I baptize you in the name of the Father, and of the Son, and of the Holy Spirit." These words must be said in order for the sacrament to be valid.
what is the MATTER?
The matter for Baptism is water. Either full immersion in water or pouring over the one being baptized is acceptable. In an emergency, the water doesn't have to be blessed or Holy.
who is the MINISTER?
The ordinary minister of baptism is an ordained member of the clergy, such as a deacon, priest, or a bishop. In emergencies, anyone can baptize as long as they do it with the correct intentions; this is only appropriate when a person may die before being baptized.
What is the rite?
Through Baptism, a new Catholic is welcomed into the Church. This usually occurs during a child's infancy, so that the parents may bring the child up strong in their faith. The minister of the Baptism either sprinkles water over the one being baptized or they are fully immersed in the water. While the child is being blessed by the Holy water, the minister says, "I baptize you in the name of the Father, and of the Son, and of the Holy Spirit." These words free the person from Original sin, as well as, any personal sins. In an emergency, anyone can administer the sacrament and the water being used does not have to be blessed.
who can receive baptism?

Anyone who is unbaptized, especially infants, can receive the sacrament of Baptism. Converts to the Christian faith can also be baptized. If someone is baptized in another non-Catholic Christian faith, their baptism is usually valid if it was done in the correct way. Although they consider themselves Christians, the Catholic Church doesn't recognize the Mormons as Christian and therefore do not accept their baptism.
how many times can this sacrament be received?
Baptism can only be received once in a person's lifetime. This is the reason why the Church doesn't re-baptize people who convert to Catholicism from another Christian faith and the Church recognizes their baptism. This Sacrament leaves an indelible, or permanent, mark on the soul of the receiver.
